Uterine fibroids and hysterectomies are hotly debated topics. 1. Since the total hysterectomy is usually performed, it leaves no uterus. This means that there will be no recurrence of fibroid tumors. Of all the surgical procedures, this is the only one that gives a permanent solution to the problem of uterine fibroids.

2. Without a uterus or cervix, the risk of uterine and cervical cancer is eliminated.

3. Unlike other surgical procedures like a myomectomy (surgical removal of only the tumors), pelvic pressure and heavy bleeding are completely eliminated.

4. Compared to a myomectomy and other surgical procedures, the hysterectomy is much easier to perform.

Uterine Fibroids and Hysterectomy — The Bad

The contentious issue with hysterectomies is that many doctors will recommend this surgical procedure without discussing many other options with the patient. In only very few cases are hysterectomies necessary such as cancer or when a woman’s life is at risk as a result of pregnancy complications, emergency bleeding, etc?

This means that in many of the hysterectomies performed each year, the surgical procedure may have been unnecessary. Why Uterine Cancer Isn’t Caught Earlier?

Over a year ago my period started on schedule. But, then it never stopped. I got online to look up “long periods” and “heavy periods” because I had always been very regular and now besides going on forever there was a lot of blood. Even clots a few times, which was scary.

The internet relieved many of my fears. I was 51 and much of my searching online led me to believe that as a woman gets closer to menopause her periods get whacky. Either skipping and getting lighter or getting longer and heavier. Either way was “normal.”

Later it did say, “If your period is lasting 3 weeks and your menses flow is extremely heavy — so heavy that it’s causing you to feel dizzy or nauseated -it might be wise to see your doctor immediately to get to the root cause and to calm your concerns.”

To calm my fears? I wasn’t dizzy or nauseated, so I was perfectly happy to choose one of their other possible reasons for my problem.

Yahoo Health reassured me even further. It stated, “Dysfunctional uterine bleeding is irregular vaginal bleeding. For example, you may get your period more often than every 21 days or farther apart than 35 days. Your period may last longer than 7 days. It is not serious, but it can be annoying and disrupt your life.

And to emedicinehealth.com; “Heavy periods are also said to be symptoms of fibroids, which most sites say aren’t harmful and many women just live with them, or they go away at menopause.” So in the worst case, I thought I was either going through menopause, had a fibroid, or was estrogen dominant and just needed some progesterone to balance me out.

None of which was any cause for alarm. But, the bleeding was getting old and since I was sure the doctor would just put me on a prescription for some progesterone to balance me out, I went ahead and scheduled my yearly “wellness physical”.

My doctor said I might just not be ovulating and put me on 10 days of progestin to stop the bleeding. She said sometimes it just needs a “kick start.” Then I could come in for a Pap test since that was included in what I had paid for the physical.

In about 10 days the bleeding stopped and I was so glad I put off the Pap test for several more weeks until the bleeding started again. It was light though so I went in anyway and she did do the Pap test.

3 weeks passed and I hadn’t heard from the office so I called. The nurse put the doctor on and she told me there were some strange cells that shouldn’t be there and I needed to come in for a biopsy to find out where they were coming from.

That was a Friday and on Wednesday I had a uterine biopsy Friday she called to tell me I had uterine cancer, but it looked like we caught it early and she was sending me to a specialist in Charlotte the following Wednesday. I went there and 1 week later he performed a hysterectomy, taking everything; cervix, uterus, and ovaries.

The next day I was home and six days later they called to tell me they had gotten everything, I would need no further treatment and should just come in for pap tests every 3 months for 2 years, then every 4 months for 2 more years and then twice the 5th year.

Normally uterine cancer is not detected by a pap test. I’m convinced that since it was done while I was bleeding that’s how they found the cells. The point though is, that even doctors don’t think abnormal bleeding is that abnormal, and women, especially as they get toward menopause can find hundreds of articles and advice forums online that will reassure them that it’s no big deal.

For me, it did end up not being that big of a deal. But, I caught it at Stage 1a. I know someone who didn’t go in until she was Stage IV and she died. People also put off going in to get checked because they are so afraid they might find out they have cancer.

I’m here to tell you if you do, it’s not the end of the world. From the day I found out I had it until the day I found out they had gotten it all was only 18 days. I like to say I only had cancer for 18 days.

The Internet is wonderful for many things. It can be a great place to gather information, but then you need to take that information to a doctor. And even then follow up!

As you can see from my story, there were many places it could have gone off track. If I hadn’t called back for my pap results when would they have called me? I also wonder about the progestin I took for 10 days because I’ve heard it can actually cause cancer.

So don’t put off checkups and screenings. Catching cancer early makes all the difference in your outcome!

Does Removing Fibroids Improve Fertility?

Fibroids are the most common benign tumor found in the uterus and may affect fertility. They may develop within the uterine wall, or they may become attached.

Symptoms of fibroids may include increased menstrual bleeding, pelvic pain, and frequent urination. Estrogen may stimulate the production of fibroids, as they may begin to show up as women enter their 20s and may not go away until after menopause.

Possible risk factors for developing fibroids include being overweight, never giving birth, and the presence of periods beginning before the age of 10. African-American women tend to have fibroids more often than other women, while heredity may also play a role in the development of fibroids.

Fertility may not be affected by fibroids that are within the uterine wall and do not change the shape of the uterine cavity, or those that bulge outside of the uterine wall. However, if fibroids change the shape of the uterine cavity or are within the cavity, fertility may be greatly decreased.

Fibroids are best detected and located using MRI. Hysterectomy and saline-infusion sonography, a procedure that includes an ultrasound following the placement of sterile water in the uterine cavity, may also be effective. Ultrasound alone, though, may not be as effective.

Fibroids that bulge into the uterine cavity or are within the cavity may cause miscarriage as well. Fibroids may thin out the lining and decrease blood flow to the embryo if the fertilized egg implants in the uterine wall near a fibroid. Fibroids may also cause inflammation that may inhibit development.

The good news is that fibroids may be removed. A procedure called Myomectomy may be performed if the fibroids are bulging into the uterine cavity or are within the cavity.

No incisions are required and the procedure is usually performed on an outpatient basis. A camera attached to a telescope that is passed through the cervix gives the doctor a visual of the fibroids. Electricity is then used to cut through the fibroids.

Hysterectomy, the removal of the uterus, is also used to treat fibroids. However, a hysterectomy should only be used if the woman has no plans to become pregnant in the present or in the future.

Most women with fibroids do not have to undergo treatment. Women whose fibroids are causing infertility or are causing other symptoms should speak with a fertility specialist regarding possible treatment options. Unfortunately, there are currently no known medications to prevent fibroids or remove them.

Infertility Heartache and Uterine Fibroids

So many women have had trouble either conceiving or being able to carry a pregnancy to full term. There are many reasons why this happens. 

First, if we rule out pelvic diseases, hormonal insufficiencies, immune system problems, medications (such as antibiotics, insulin, and antidepressants), scar tissue, and premature menopause, then other causes could explain the difficulty. With all the things that could either be wrong or go wrong, it is a wonder people get to be born at all!

But say we’re dealing with a perfectly healthy woman with regular menstrual cycles, and normal production of fertile eggs, and the male has a high sperm count, but fertility is still an issue. There are causes that are not the first to be considered. One cause often overlooked is the presence of uterine fibroids.

These are considered to be benign tumors of the uterine muscle (that is if you view infertility to be benign). However, it’s the size and location that are crucial here. Some fibroids are small and located in an area of the uterus that won’t have any impact on the reproductive process.

The 3 usual locations for fibroids are the Subserosal which is on the outside surface of the uterus,

Intramural which is within the muscular wall of the uterus, and the last ones are the Submucous fibroids which bulge into the uterine cavity. This one is the problematic kind of fibroid because it is located inside the uterine cavity.

That’s not to say that fibroids that are situated near the fallopian tubes or cervix aren’t a problem. Those prevent the sperm or even the fertilized egg from reaching the uterus and implanting there. Sometimes they can even block the fallopian tubes causing no eggs to be able to get fertilized. Bottom line: uterine fibroids are bad.

But getting back to the submucosal fibroids. Large ones can disrupt the uterine lining as I mentioned before, making it difficult for proper implantation. They can also get in the way of the growth of an embryo which would increase the chances of miscarriage.

If the fibroids grow even a little bit under the endometrial lining in the uterus and then protrude into the uterine cavity, they can cause fetal deformities, premature labor, and other labor complications. This is just so awful.

Before trying to conceive, uterine fibroids are detectable by a pelvic exam or better yet a pelvic sonogram. But what if your doctor suggests surgery to remove the fibroids before trying to get pregnant (if you’re lucky enough to find out before conception)? You should consider the fact that there can be a lot of blood loss and then of course the time needed for recovery to be ready to try to get pregnant again.

Some surgeries, such as a myomectomy claim to be minimally invasive, but then they have to reconstruct the uterus after they remove the fibroids. Does that sound minimally invasive? Not to me, it doesn’t. Not only that, but sometimes new fibroids can grow back later on.

Women in other countries around the world have all had to deal with fertility obstacles also, though statistically, not as much as in the United States. The difference is in their approach to a woman’s overall well-being and having the body function as a healthy organism as a whole.

Their approach is more of a maintenance of uterine health to prevent these problems in the first place. You already know if you have some kind of gynecological problem.
